CAPCare

CAPCare is operated by Rowan Regional Medical Center. It is a "community alternatives program" that offers the elderly and disabled an opportunity to continue living in the comfort and security of their own homes.

"Our goal is to help elderly and disabled adults continue leading their lives within the community with dignity and independence."

CAPCare clients receive appropriate medical care and support services at home, a safe alternative to placement in a nursing facility. For many elderly and disabled adults today, CAPCare is the answer.

An Individual Plan of Care
Services provided by CAPCare are part of an overall plan of care developed for the individual needs of each CAPCare client. Your particular needs will be assessed during a home visit from a case management team - a social worker and a nurse under the supervision of your doctor. A plan of care is then carefully designed wit hregard to an individual's needs for health, safety and well-being.

Once an individual plan of care has been developed, your case manager will arrange all necessary services. Your case manager will continue to monitor and evaluate these services.

Dignity and Independence
CAPCare is operated by Rowan Regional Medical Center. Our goal is to help elderly and disabled adults continue leading their lives within the community with dignity and independence. This goal can be accomplished by providing essential services to clients in their own homes.

Eligibility Requirements
The CAPCare program is intended for residents of Rowan County who may be facing nursing home placement because of disability or chronic illness. You must be in need of medical attention and/or support services to remain safely and comfortably at home, and your personal physician's approval is required. In addition, CAPCare clients must meet Medicaid eligibility requirements determined by the Department of Social-Services.

Payment
CAPCare services are reimbursed by Medicaid. Based onyour income, the Department of Social Services will determine if you will be responsible for a portion of the cost of your CAPCare services.
